Sleeve Gastrectomy - Banded and Conventional

There are two ways a sleeve gastrectomy can be performed-conventional and banded.

• Conventional Sleeve Gastrectomy
  It is performed laparoscopically. After the introduction of general anesthesia, several small incisions are made in the abdomen through which trocars are passed. The trocars allow the surgeon to introduce a camera, carbon dioxide gas, and other instruments needed for the procedure. A large chunk of the stomach is surgically removed, reducing its size to about 10% of the original size. The new stomach pouch that looks like a banana has a reduced capacity. So, the person naturally eats less food than he normally would. The surgery has gained popularity due to its safety and effectiveness, shorter pre-operative duration, and feasibility of revision and conversion to more extensive surgery. However, there are higher chances of weight regain after the surgery secondary to dilatation of the stomach pouch. So, a banded sleeve gastrectomy is a much more efficient option.
• Banded Sleeve Gastrectomy
  This procedure involves the introduction of a silastic ring of 7.5 cm diameter through one of the laparoscopic trocars and wrapping it around the upper part of the stomach sleeve. This stomach banding increases the success rate of the operation by preventing gastric tube dilatation. Therefore, weight loss is maintained in the long term.

The biggest complication of a conventional sleeve gastrectomy is dilatation of the gastric pouch over time. This results in inefficient weight loss or weight regain after the surgery. Other complications include infection, bleeding and leaks from the anastomotic site.
